WABASHA COUNTY DAC JOB DESCRIPTION
Direct Support Professional/Job Coach
The responsibilities of the Production Assistant at the Wabasha County DAC are as outlined but not limited to the following:
Conduct shall always meet high professional standards, including confidentiality, co-worker relations, cooperation and team work. Perform all responsibilities including additional responsibilities not outlined below when designated by your supervisor. Job duties may be subject to change as needed to fulfill State requirements and/or the successful operations of the DAC.
The Wabasha County Developmental Achievement Center is a Day Training and Habilitation facility that supports individuals with mild to severe mental and physical disabilities. As a team member, you are expected to assist in providing services that develop and maintain life skills for the individuals we serve so they can achieve greater independence, productivity, and community inclusion.
DT&H services provide training, supervision, and support in the areas of:
• Communication
• Community access, mobility, and inclusion
• Money management and budgeting
• Pedestrian / street safety
• Positive Behavior Support
• Public transportation
• Recreation and leisure
• Self-care
• Self-preservation
• Socialization
• Use of various community resources
• Work related skills training
GENERAL DUTIES:
A. Train, supervise and support individuals in developing skills to successfully communicate their needs and wishes. Encourage non-verbal individuals to utilize alternative methods based on their Coordinated Service and Support Plan. Encourage individuals who have communication barriers to use words to successfully and appropriately communicate their needs and wishes.
B. Train, supervise and support individuals through functional and daily life skills such as: reading, money, time, concepts, writing, basic math, communication, and computer skills through the use of technology.
C. Train, supervise and support individuals in the use of community resources that may benefit their daily lives outside the DAC including pedestrian traffic safety, directions to desired destinations, abuse prevention while in the community.
D. Train, supervise and support individuals to improve their overall physical health through cardiovascular fitness programs, balance, strength, ambulation, range of motion, and endurance. Work with individuals in experiencing sight, hearing, touching, smelling, and tasting sensation activities that promote interest and excitement in developing these skills.
E. Train, supervise and support individuals through life enrichment activities including practicing for and experiencing social situations, displaying appropriate interactions with others, instilling interest in constructive leisure activities like crafts and hobbies to improve fine motor abilities, finger dexterity and hand-eye coordination.
F. Train, supervise and support individuals in developing ways to successfully maintain self-care and personal hygiene and bathroom techniques, appropriate dress for work, leisure activities and weather, and choosing healthy dietary choices.
G. Train, supervise and support individuals in modeling and reinforcing appropriate actions, discouraging conduct that is detrimental to oneself and others, experiencing choice, problem solving, decision making, reinforcement and consequences.
H. Train, supervise and support individuals through the DAC work program:
1. Assist with job support, exploration and development to individuals who express a desire for gainful employment to discover what jobs are in the community, what jobs look interesting and what skills are needed to obtain certain jobs.
2. Support individuals who’s programming involves DAC based sub-contract work including cleaning, laundry, packaging and assembly, mailings, making badge buttons, gift bows and other crafts, document shredding and aluminum can recycling.
3. Represent the DAC at enclave or community-based contracted employment by being on time, polite, professional and respectful while at job sites and encourage individuals served to do the same. Demonstrate strong leadership skills and provide assistance and guidance for those who may be struggling performing their assigned duties.
ASSISTING INDIVIDUALS WITH SPECIAL NEEDS:
A. Provide assistance and support to individuals with disabilities that have impaired their ability to independently perform personal hygiene and self-care independently. This includes assistance with:
1. Toileting, changing soiled incontinent products and assisting with clothing changes, monitoring individuals who wander so they can get to the restroom and return to programming safely, and encouraging the practice of good hygiene; encouraging hand-washing, assisting with good oral hygiene such as brushing teeth after meals, etc.
2. Prepare snacks during break times and preparing lunches for individuals requiring assistance. Encouraging self-feeding through adaptive aids; specially designed tableware, drinking glasses, G-tube and Ileostomy cares.
3. Assist individuals with ambulation needs and those at risk of wandering safely arrive and depart the DAC.
4. Provide assistance with adaptive transfer aids and equipment such as lifts, commodes, therapy mats, special needs beds and table, etc.
5. Once trained, provide medication assistance and/or supervision for individuals requiring this service at the DAC.
ASSISTING INDIVIDUALS WITH POSITIVE SUPPORT PLANS:
A. Provide assistant and support to individuals that have developed a Positive Support Services Plan with their interdisciplinary team. This includes:
1. Providing 1:1 support for individuals exhibiting challenging behaviors.
2. Comply with Minnesota 245D Statutes regarding the Positive Support Rule and DAC policies and procedures regarding physical intervention alternatives.
3. Provide assistance during transport for individuals who may cause safety concerns or when medical conditions may need monitoring while in a vehicle.
4. Be knowledgeable of the all positive support plans and implement them as directed. Do not alter any plans without consulting the DAC Positive Support Supervisor or Executive Director.
COORDINATION, SUPERVISION, AND MANAGEMENT OF DAC WORK CREW:
1. Assist the Production Manager in establishing and implementing a coordinated service support plan addendum (CSSPA) for clients scheduled in the Work Component.
2. Help plan and develop workstations and projects to maximize each client’s potential work ability.
3. Supervise clients in maintaining a high level detail and accuracy with cleaning and/or production work
4. Re-evaluate clients’ production percentages quarterly.
5. Submit timecards to the office daily.
6. Report to the Director all accidents as soon as possible.
GENERAL EXPECTATIONS:
A. You are a Mandated Reporter. You must follow all rules and regulations outlined in the Vulnerable Adult Act, Reporting and Review of the Maltreatment of Minors and the DAC’s Program Abuse Prevention Plan.
B. You will review all Persons Served files after each individual has their annual conference. This includes review of their Coordinated Community Support Plan, Self-Management Plan, Individual Abuse Prevention Plan and Skills Needs Inventory.
C. Report non-typical occurrences or behaviors, medical or health needs to your direct supervisor and/or administrative staff so proper documentation and communication can be relayed to other DAC staff, residential caregivers, guardians and case managers can be made aware if necessary.
D. Be aware of all persons served medical and psychological health, including allergies, choking risks, healthcare directives, etc.
E. Respect Service Recipient Rights and Data Privacy.
F. Be knowledgeable of all emergency protocols, including Basic First Aid.
G. Report all accidents to the Executive Director or management staff as soon as possible.
H. Be knowledgeable of all 245D Regulations and DAC Policies and Procedures.
I. Participate in positive public relation efforts on behalf of the DAC including attending fundraising events, promotional campaigns, etc.
J. Take responsibility for personal and professional growth by attending all required team member meetings and trainings.
